How many hyaluronic acid (HA) dermal filler syringes do I need?

Great question! First, you would need to seriously ask yourself what your personal beauty goals are. Are you looking for a slight enhancement, moderate or a major change? Dermal fillers, like Restylane and Juvederm, are used to increase volume and to fill in lines and wrinkles.  Are you expecting your wrinkle to be relaxed? If so, stay tuned for the next blog that discusses Botox vs. Dysport. Toxins are used to relax wrinkles, typically in the upper face. An experienced cosmetic injector also knows advanced toxin injection techniques for the lower face.

Slight Enhancement

If you are looking for a slight enhancement like a few fine lines, slight lip plumping, or a little hollowness filled in, then you likely will only require a small to moderate amount of HA dermal filler. This can easily vary from 0.5mL-3mL approximately, however, each patient should be assessed by their provider to determine their personalized beauty and aesthetic goals.

Moderate-Major Enhancement

Upper Face

Maybe you are middle aged, (gasp!!!), and the upper and mid face are starting to flatten and lose the volume it once had 10-15 years ago. The upper face includes the forehead and glabella (mid brow, aka the “11’s”) region. Sometimes wrinkle relaxer such as, Botox or Dysport, are not enough to obtain the desired results people are requesting. Once the toxin is in full effect, we can add small amounts of dermal filler to “pop” out fine lines that remain. Total Upper Face Liquid Lift 1-2 mL.

Mid Face

The mid face includes the under eye area/tear trough, temples, cheeks, and nose. These areas require a considerable amount of HA dermal filler. Tear trough could easily require 0.5-1mL per side; the temples 0.5-2 mL per side; cheeks 1-3 mL per side (yes up to 6 mL total just to lift your flattened cheeks); the nose usually would require less filler secondary to the surface area, 0.5-1.0 mL. Total Mid Face Liquid Lift: 5-10 mL

Lower Face

The lower face includes the nasolabial folds, marionette lines, oral commissures, lip region (lip lines, lip boarder and to also to plump the lips), and lastly the jaw line. The nasolabial folds are enhanced and get deeper over time when the cheeks drop and flatten over time from loss of volume. After volume is replaced (if necessary) to the cheeks, these lines can be filled in with as little as 0.5 mL up to 2 mL per side. The marionette lines could require 0.5 mL-1mL per side. Oral commissures are located in the “corner” of the mouth, these start to turn down over time. To reverse this issue 0.25 mL-0.5 mL could easily be used. Lips!! I love injecting the lips, however, many people misunderstand the actual amount of product required to reach their desired results. Lip lines (aka smokers) lines may take 1-2 mL to correct, the deeper the lines then the more they require. Lip boarder can take 0.5 to 1 mL. The actual lips may require 1-2 mL. Yes, you read that correctly, the entire lip area could easily take 5 mL to correct fully. Now, not EVERYONE will require that much product, but if you are or were a chronic smoker for most of your adult life, you very likely will require much more product than the non smoker. Jaw line may take 1-3mL, depending on your desired outcomes. Total Lower Face Liquid Face Lift: 5-11 mL

Please know dermal filler can easily be inserted slowly over time and gradually receive additional product. I like to refer to this as “layering”. This is a safe way to receive conservative facial augmentation without the risks involved during plastic surgery. I prefer to add 1-3 mL of HA dermal filler every 3-4 months during my patients toxin/wrinkle relaxer appointments. Once a patients beauty goals are met, we develop a maintenance plan.

All patients new to receiving dermal filler should understand recovery time can be as little as a few days up to 4 weeks or longer. It is normal for one side of your face to heal before the other side. Some “unevenness” is a normal occurrence and will resolve over the healing period. Remember, your facial hollowness, lines, and deficits are being poked several times with a needle. This causes natural reaction of swelling (edema) just from the trauma of the needle, in addition, your surface area is being expanded with the addition of the HA dermal filler product. Normal healing side effects include tenderness, bruising, and swelling. You would not expect to be healed the day after or even 1 week after having surgery, right? Then allow yourself the proper amount of recovery and healing for your in-office aesthetics procedure, before making the decision if you love or hate anything. It is a gradual process. It must be respected.
